Download and fill out the appropriate files for your session. There is a short description and what each one is for. Email, call, or text us if you have any questions.
Longest file. Basic contract that identifies FDP as the exclusive photographer and overview on how everything is handled

This is a contact sheet for our records. Please fill this out and return it to us. We will never sell or release your information.
This is the release allowing FDP to take and use photos of the models, subjects, or participants.
Basically the same as the other file with a couple of changes specific to kids.
